Not very common. The rules said they were supposed to be closed before movement... but of course we all know that rules are often broken. In addition, most the door hardware used since 1900 or so would latch a door if slack action should slam it closed. By of course, we've all seen open boxcar doors; sometimes, in the case of plug doors, under bridges after the bridge structure sheared them off.
