You stand and face the B end of the car. The side to your right is the Right side and to your left is the left side. Also the journals number from the B end of the car R1 R2 R3 R4 and L1 L2 L3 L4 on

Des Plaines Hobbies had Canadian style ladders done with
an in-line stirrup for 10'0" box cars.

Tichy has some. You can also buy Intermountain parts separately, and their PS-1 ladders have attached steps.
