As Tom C said, I use a 2-56 set screw.
If the centre raised collar is damaged/missing or the thread has pulled
out/been stripped, I remove the existing collar and drill the bolster with a
1/8 drill

HO Truck Bolster Clearance Hole
Another thought on this problem:
If you would use a flat headed screw it would solve all of those
problems.
The cone of this head acts as a centering device.
It doesn't matter what size the hole

I've been using 2/56 screws with all of my trucks with no problem. I've
never heard of sanding the pads off the bottom of the car and putting a cut up KD
box in it's place.

There is another way to do this: Make a thin "washer" which fits inside the "cup" of the bolster,
and in the center of that make a clearance hole for 2-56. Then put this on the screw shaft
